When your child’s exam is complete you will be given an encounter form that will note the type of exam, procedures and tests for which you will be charged. Please take the form to the check-out window for processing.
In order to file your insurance we must see your child’s insurance card at each visit. If you do not have the necessary information and we are unable to verify your coverage, we must ask for payment at the time of your visit. We also require all co-payments be paid at the time services are rendered.
We accept personal check, check card, Visa and Mastercard. Please note that there is a $30.00 fee charged for returned checks.
A monthly statement will be sent to you detailing unpaid charges. If you have any questions regarding services not paid by your insurance we ask that you contact your insurance company or employer.
